- R5 Extension Pack published with sex and gender extensions
- Pre-adopt R5 extensions requires some work. Cannot just adopt the R5 FHIR Extensions Pack as a dependency in R4 AU Base. There are backwards incompatible data type issues and terminology conflicts.
- Alternative option is to 'copy' of extension, (e.g. http://hl7.org/fhir/extensions/1.0.0/StructureDefinition-patient-sexParameterForClinicalUse.html), and the referenced terminology and publish in HL7 AU Base.
- New extensions Patient Sex Parameter For Clinical Use and Person Recorded Sex Or Gender use an incompatible data type with R4 - the CodeableReference data type

- An option to deal with data type incompatability is to 'convert' to R4 when publishing in HL7 AU Base - leaves the question of a canonical conversion for CodeableReference
- does the extension element need to be CodeableReference or could the extension be changed to backwards compatible choice of CodeableConcept | R4
- in converting do we need to support both or given the use of similar R4 elements can it just use Reference in the converted extension?
- will HL7 publish R4 compatible versions of the new extensions have been added in the R5 Extensions Pack?

- On use of Person Recorded Sex Or Gender extension:
- this extension is very early ad only FMM 1 Draft in extension - some concern in HL7 AU on recommending it's use in AU Base
- discussion on where to include it - discussion on not including on Patient; preference is for use in Clinical resources (e.g. the context-specific instruction for that prescription or diagnostic test)
