   1. Observation.code.coding:loincSexAtBirth.code is fixed to 76689-9; which has the definition: "The sex that was assigned and recorded on the birth certificate at the time of an individual's birth." Does it matter that it is specific to the birth certificate?
   2. Observation.effective[x] is constrained to only have datatype dateTime (excluding Period, Timing and instant). On face value, this would make sense, but seeking verification given HL7 AU's AU Base principle to provide guidance not constraints.
      
   3. Observation.performer has no constraints in the profile, but the core spec allows the performer to be Practitioner | PractitionerRole | Organization | CareTeam | Patient | RelatedPerson. Consider constraining out Patient - or is that overkill?
